29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: At the worksite located at 400 SE 18th Street, Oak Grove, Missouri: Employees were exposed to fall hazards when using a portable ladder to egress an excavation. Employees used a 6-foot A-frame metal ladder to access/egress the excavation. The ladder did not extend 3 feet above the top of the excavation.
