29 CFR 1910.263(f)(3): Rear cover(s) of the divider(s) were not provided with a limit switch to prevent machine operation while the cover(s) were open: a. On or about February 4, 2025, Production Department, the employer failed to provide a limit switch on the Werner & Pfleiderer Divider on Line 3 that would prevent machine operation while the cover was open. An employee was injured while attempting to remove a jam and the cover was able to open three inches before the machine stopped, thereby the employee received a fingertip amputation.
