29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees. (a) In the kettle room - On or about October 2, 2024, the employer exposed employees to struck-by hazards, in that, the employer did not ensure lockout devices were affixed to each energy isolating device associated with the heat exchangers before allowing employees to open system pipes.
