29 CFR 1926.1204(h):(h) Designate each person who is to have an active role (as, for example, authorized entrants, attendants, entry supervisors, or persons who test or monitor the atmosphere in a permit space) in entry operations, identify the duties of each such employee, and provide each such employee with the training required by �1926.1207; (a) On or about December 3, 2024, and at times prior, at 925 19th Ave NW, Birmingham, AL 35215, Gulf Coast Underground, LLC failed to designate roles per person who assisted in or entered the permit space while employees were working in an active sewer system. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
