29 CFR 1910.147(d)(4)(ii):Lockout devices utilized were not affixed in a manner that held the energy isolating device in a safe or off position: On or about October 28th, 2024, employees engaged in maintenance operations located at 4657 G Street, Omaha, Nebraska 68117 were exposed to pneumatic and thermal energy source hazards in that the employer failed to require employees to affix their LOTO devices on the Blue Oven prior to commencing work on it.
