29 CFR 1926.451(g)(3)(iv): Vertical lifelines, independent support lines, and suspension ropes shall not be attached to each other, nor shall they be attached to or use the same point of anchorage, nor shall they be attached to the same point on the scaffold or personal fall arrest system. (a) On or about October 10, 2024, and at times prior, at 901 North 9th Street, Milwaukee, WI 53203, the employer had not verified that each vertical lifeline were independent of one another and/or attached at the same anchorage point, in-that the lifelines were secured to a single steel cable that wrapped around the parapet wall on the north side of the building. (b) On or about October 10, 2024, and at times prior, at 901 North 9th Street, Milwaukee, WI 53203, the employer had not verified the vertical lifelines were independent of the scaffold system in that the steel cable that held the lifelines and the steel cable that held the tiebacks were connected with a Crosby 3/4" D-ring shackle.
