29 CFR 1910.184(i)(1): Synthetic web sling(s) were not marked or coded to show rated capacities for each type of hitch and type of synthetic web material: On or about October 7, 2024, it was identified that employees engaged in pretensioning activities at Coreslab Structures (Omaha) Inc. located at 802 Alliance Road, Bellevue, Nebraska were exposed to struck-by hazards in that identification markings were not legible on the synthetic web sling supporting the hydraulic jacking ram. Instances included, but were not limited to the synthetic web slings used during: i. Daily operations in building 1 where employees used the hydraulic jacking ram; G. T. Bynum CO. Model M P U28, Serial number: 122297 and ii. Daily operations in building 2 where employees used the hydraulic jacking ram; G. T. Bynum CO., Model: M P U2, Serial number: 111396.
