29 CFR 1910.333(c)(2): The employer did not ensure that only qualified persons were working on or near energized equipment and that qualified persons were familiar with the proper use of special precautionary techniques, personal protective equipment, insulating and shielding materials, and insulated tools a. High Voltage Testing - On or about April 23, 2024, employees were performing high voltage testing, this requires setting up and troubleshooting high voltage equipment. The testing cell where the high voltage equipment was being housed and operated did not shield or separate unqualified persons to a safe distance given the voltage of equipment with ranging from 35kv transformer near the edge of the cage and a resonator testing up to 400kv. b. High Voltage Testing - On or about August 20 and 21, 2024, employees were performing high voltage testing, this requires setting up and troubleshooting high voltage equipment. Designated qualified persons were entering and being exposed to high voltage in a live state (not locked out), where employees entered into the cell closing the door reconnecting the interlock safety system without the necessary protective equipment or shielding to be exposed to high voltage equipment. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records
