29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii): a) 4701 Pine St, Philadelphia, PA 19143 - On or about April 11, 2024, the employer did not ensure each platform on all working levels of scaffolds was fully planked or decked between the front uprights and the guardrail supports. Employee performed masonry facade work on the front (southern side) of the building from a scaffold platform that was not fully planked, thereby exposing employees to fall hazards of approximately 6 feet. b) 4701 Pine St, Philadelphia, PA 19143 - On or about April 11, 2024, the employer did not ensure each platform on all working levels of scaffolds was fully planked or decked between the front uprights and the guardrail supports. Employee performed masonry facade work on the west side of the building from a scaffold platform that was not fully planked, which lacked three planks, thereby exposing employees to fall hazards of approximately 13.5 feet.
