29 CFR 1926.416(a)(1): Employee(s) were permitted to work in proximity to electric power circuits that were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: a) On or about April 4, 2024, at the above addressed jobsite, employees permitted to work in cabinets of a variable frequency drive were in proximity to energized electric power circuits with voltage of 7403.47 volts and were not protected against electric shock, thereby exposing employees to electrical hazards. b) On or about April 4, 2024, at the above addressed jobsite, employees were in proximity to energized electric power circuits with voltage of 480 volts and were not protected against electric shock, thereby exposing employees to electrical hazards.
