29 CFR 1926.454(b): The employer did not have each employee who was involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work. Jobsite: On or about December 12, 2023, the employer did not ensure that employees who erected a ladder-jack scaffold approximately 19 feet, 8 inches from ground level were trained by a competent person. A fall protection system was not included as part of the erection plan.
