29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l): On or about January 8, 2024 employees engaged in cold storage activities at United States Cold Storage located at 4302 South 30th Street, Omaha, Nebraska were exposed to crushing and struck-by hazards in that the employer failed to certify each operator received refresher training including evaluation of the effectiveness of that training. Instances included but were not limited to: i. Operator(s) involved in an accident or near-miss incident and, ii. Evaluation of each powered industrial truck operators' performance at least once every three years.
