29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle: On or about December 14, 2023, at a facility located at 246 Trowbridge Drive in Fond du Lac, WI; point of operation hazards presented by the Packaging Aids Corporation bag sealer (Model PVS-GA36, S# GA94120-170961) were not guarded. The foot pedal actuated device did not have means to prevent employees' hands from entering the danger zone during the operating cycle exposing employees to caught-between and thermal hazards.
