29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: (a)DHD Construction, Inc. dba Wall Building Center at 19775 223rd Street, Quinn, SD 57775: On or about and at times prior to September 13, 2023, the employer did not ensure that training was conducted for employees prior to using a Genie aerial lift platform, model number S-45, while conducting roofing activities on a commercial building. This condition exposed employees to fall and ejection hazards while working at heights greater than 10 feet.
