29 CFR 1910.334(a)(3)(ii):Attachment plugs and receptacles were connected or altered in a manner which prevented proper continuity of the electric equipment grounding conductor at the point where plugs were attached to receptacles: 5914 Zangs Drive, San Antonio, TX.: On or about September 13, 2023, and at times prior thereto, employees were exposed to electrical hazards due to a relocatable power tap plugged into an orange extension cord that was plugged into an energized outlet, preventing continuity of the power tap grounding capabilities.
