29 CFR 1910.119(e)(3)(iii): The process hazard analysis did not address engineering and administrative controls applicable to the hazards and their interrelationships such as appropriate application of detection methodologies to provide early warning of releases: (a)Ring-neck Energy & Feed, LLC dba Ringneck Energy LLC at 901 Redwood Avenue in Onida, SD 57564: On or about and at times prior to July 9, 2023, the employer's 2019 Process Hazard Analysis failed to address the need for an effective flammable vapor detection system within the operations building, where the control room is located. In this case, flammable vapors were vented from the slurry tank and accumulated in the operations building ultimately contacting an ignition source and caused a explosion and fire.
