29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied: (a)Core Framing, LLC at 5501 - 5503 & 5505 - 5507 South Huntwood Avenue, Sioux Falls, SD 57110: On or about and at times prior to July 13, 2023 , the exposing employer did not ensure that the elevated platform was properly attached to the Terex All-Terrain Forklift, model TH1056C while occupied during framing operations. This condition exposed employees to fall and crushing hazards associated with the platform separating from the mast of the forklift. (b)Core Framing, LLC at 5501 - 5503 & 5505 - 5507 South Huntwood Avenue, Sioux Falls, SD 57110: On or about and at times prior to July 13, 2023 , the exposing employer did not ensure that the elevated platform was properly attached to the JLG All-Terrain Forklift, model 6042 while occupied during framing operations. This condition exposed employees to fall and crushing hazards associated with the platform separating from the mast of the forklift. (c)Core Framing, LLC at 5501 - 5503 & 5505 - 5507 South Huntwood Avenue, Sioux Falls, SD 57110: On or about and at times prior to July 13, 2023 , the exposing employer did not ensure that the elevated platform was properly attached to the JLG All-Terrain Forklift, model G6-42A while occupied during framing operations. This condition exposed employees to fall and crushing hazards associated with the platform separating from the mast of the forklift.
