(2) Is secured to prevent accidental displacement. a) On or about August 25, 2023, in the Rag Room at rotary #1 and 2, the employer did not ensure that the plywood cover over the floor holes at the rotaries were secured to prevent accidental displacement as the employer was relying on carpeted edges to secure the plywood in place when the rotaries were cooking, emptying, or not in use.
