29 CFR 1910.147(e)(2)(i): Release from lockout or tagout. Before lockout or tagout devices are removed and energy is restored to the machine or equipment, procedures shall be followed and actions taken by the authorized employee(s) to ensure the following: The work area shall be checked to ensure that all employees have been safely positioned or removed. a) On or about June 15, 2023, the employer did not ensure that the work area was checked to ensure employees were in a safe position prior to jogging the Disamatic 3 machine, resulting in an employee's arm being caught between the drive arm and the metal beam support.
