29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): Galion, LLC located at 515 N. East Street in Galion, Ohio: On or about February 14, 2023, the employer did not ensure that each operator was competent to operate powered industrial vehicles demonstrated through successful completion of training and evaluation requirements. Failure to ensure employees are competent to operate powered industrial vehicles exposed an employee to struck-by hazards while operating the PIV near the CNC room.
