29 CFR 1910.119(j)(5): The employer did not correct deficiencies in equipment that were outside acceptable limits (defined by the process safety information on paragraph (d) of this section) before use: (a) Building 1, trains 6/7 (Ethanothebaine crude wet process), hydrogenator H-1024: On or about 12/22/22 until 1/19/23, the employer did not correct a flange leak occurring at the bottom valve of vessel H-1024,exposing employees to fire, explosion, and toxic hazards during operation.
