29 CFR 1910.178(l)(4)(iii):An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: The employer is failing to protect employees from struck-by, caught under, and crushing hazards. This was most recently documented on January 10, 2023, at the worksite located at 299 Extension Street, Verona, Missouri. Employees were operating forklifts, manufactured by Doosan and/or Toyota, throughout the plant and did not have an evaluation of their performance at least once every three years.
