29 CFR 1910.184(c)(9):Employee(s) were not kept clear of load(s) suspended, or about to be lifted by sling(s): a. Ultimate RB, Inc. - Delphos, Ohio: On or about December 13, 2022, the employer did not ensure that employees were kept clear of loads about to be lifted, and/or of suspended loads, when using an overhead hoist with chain and nylon slings to lift a press platen(s). Employees were exposed to struck-by hazards in the event of material displacement. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
