29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one-fourth inch: (a)Bell Incorporated at 609 West Algonquin Street, Sioux Falls, SD 57104: On or about and at times prior to November 30, 2022, the employer did not ensure that the Dayton Pedestal Industrial Grinder, Model # 2Z341N being utilized within the maintenance area had an adjustable tongue installed to cover the top maximum angular exposure above the horizontal plane of the wheel spindle projections. This condition exposed employees to struck-by hazards associated with grinding wheel breakage.
