On or about November 28, 2022 and times prior, the employer did not ensure that the entry supervisor verified that the appropriate entries were made on the permit, that all tests specified by the permit were been conducted and that all procedures and equipment specified by the permit were in place before endorsing the permit and allowing entry to begin by the authorized entrant to start the tank cleaning work in the confined space.
