29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: On or about October 28, 2022, and at time prior thereto, the employer's SOP(s), Standard Work Visual Instructions also referred to as Inch Safe procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing the following equipment: a) Checking Surveillance System - The SOP does not include how to lock out all the energy sources. It only indicates "with stop button down". b) Start-up - The SOP does not include how to lock out all the energy sources. It only indicates "Start machine and run to 200 rpm Speed". c) Patch Hoist - The SOP does not include how to lock out all the energy sources. It only indicates "stop the machine... with the stop button down". d) Pulling out 'Out of Tolerance' envelopes. It only indicates "machines need to be shut down"
