29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: a) Worksite - On or about August 29, 2022, two employees were exposed to tipping and fall hazards when working on top of a scaffold (WEN Scaffolding, Model: 31110) stacked 3 high with each scaffold stack measuring approximately 65 inches in height and the wheel height measuring approximately 6 inches. The two employees were working from a height of approximately 16 feet 8 inches without any guardrails in place.
