29 CFR 1910.147(f)(1)(ii): In situations in which lockout or tagout devices must be temporarily removed from the energy isolating device and the machine or equipment energized to test or position the machine, equipment or component thereof, the following sequence of actions shall be followed: Remove employees from the machine or equipment area in accordance with paragraph (e)(2) of this section; a) On July 26, 2022, the employer did not ensure that employees were first removed from the machine area before energizing the machine to facilitate testing and positioning of the lower wrapper arm on coiler #2. As a result, an employee was exposed when the bottom pneumatic wrapper arm was activated. b) On or about July 19, 2022, the employer did not ensure that employees were first removed from the machine area before energizing the machine to facilitate testing and positioning of the spreader/side guides on PR3 Entry. c) On or about July 25, 2022, the employer did not ensure that employees were first removed from the machine area before energizing the machine to facilitate testing and positioning of the Polishing Sled on the coiler roll lines.
