29 CFR 1910.147(c)(7)(i):The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees: a) Plas-Tec Corp. on or about July 13, 2022, at the address of 601 W. Indiana St. Edon, OH: The employer did not adequately train employees to lock out/tagout equipment such as but not limited to #13 plastic injection machine. Employees did not control all hazardous energy sources such as, but not limited to thermally pressurized molten plastic. Employees performing maintenance/servicing activities such as but not limited to clearing a blocked injection nozzle, without the benefit of energy control, were exposed to burns from pressurized molten plastic.
