29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l): The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation: (a)Forest Products Distributors, Inc. at 5406 Old Folsom Road, Rapid City, SD 57703: On or about and at times prior to May 12, 2022, the employer did not ensure employees were protected from stuck-by hazards in that not all forklift operators were certified by the employer as trained and evaluated as required by paragraph (l) prior to operating forklifts for production. This condition exposed employees to struck-by hazards associated with improper forklift usage.
