29 CFR 1910.119(j)(5): The employer did not correct deficiencies in equipment that were outside acceptable limits (defined by the process safety information in paragraph (d) of this section) before further use or in a safe and timely manner when necessary means were taken to assure safe operation: On or about May 9, 2022 in the refrigeration engine room at Gordon Foods in Houston, Texas, employees were exposed to struck-by hazards and chemical hazards of anhydrous ammonia when pressure safety valves that had been installed in the process for more than five years were not replaced, and continued to be relied upon for pressure protection of equipment in the ammonia refrigeration process. a) The PSV to the oil cooler on compressor TSOC2 was not replaced after five years of being installed and in service. b) The PSV to the oil cooler on compressor TSOC3 was not replaced after five years of being installed and in service. c) The PSV to the oil cooler on compressor TSOC4 was not replaced after five years of being installed and in service. d) The PSV to the oil cooler on compressor TSOC6 was not replaced after five years of being installed and in service.
