29 CFR 1910.147(d)(5)(i): Following the application of lockout or tagout devices to energy isolating devices, all potentially hazardous stored or residual energy did not relieve, disconnect, restrain, and otherwise rendered safe. On or about April 23, 2022, in the ROSE unit at Valero Meraux refinery located in Meraux, LA, workers were exposed to fire and explosion hazards, when the employer did not relieve and dissipate hazardous residual energy from 8-inch vertical check valve (CC-013), its associated piping and vessel V-102.
