29 CFR 1910.140(c)(18): The employer did not ensure that personal fall protection systems are inspected before initial use during each work shift for mildew, wear, damage, and other deterioration, and defective components are removed from service: On or about March 28, 2022, the employer did not ensure that the personal fall protection systems were inspected before initial use during each work shift when employees utilized a horizontal lifeline system when servicing the overflow sensors and vents on the top of tanker trucks exposing employees to a fall hazard of approximately 9 feet.
