29 CFR 1910.179(j)(2)(iv): Monthly inspections of hoist chains, with a certification record which includes the date of inspection, the signature of the person who performed the inspection and an identifier of the chain which was inspected, were not performed: a. On or about March 24, 2022 at 200 West Railroad Street, Columbiana, OH 44408, the employer failed to ensure the chains/wire rope on CM 2-ton crane, serial number WR3G2294QB, model number WRMT2028 3, was inspected and documented monthly in that deficiencies identified by a third party company were not repaired. b. On or about March 24, 2022 at 200 West Railroad Street, Columbiana, OH 44408, the employer failed to ensure the chains/wire rope on the R&M 5-ton crane, model number F3M, serial number 32095DB1, was inspected and documented monthly in that deficiencies identified by a third party company were not repaired.
