29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not conducted at least once every three years. a.) Warehouse- 100 Electric Avenue, Secaucus, NJ 07094 The employer did not ensure employee operating powered industrial equipment were evaluated at least once every three years. An employee operated the LP power industrial truck was not evaluated after three years of receiving his certification to ensure the safe operation of the forklift he utilized daily. Violation occurred on or about February 11, 2022.
