29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: (a) Marine Tech Wire & Cable Inc., 631 South Richland Avenue, York, Pennsylvania - The employer did not conduct an evaluation of each powered industrial truck operator at least once every three years, and the employer permitted these employees to operate powered industrial trucks, such as the Toyota sit-down rider forklift, exposing employees to hazards including, but not limited to, tipping of a forklift or struck-by a forklift, on or about February 9, 2022. No abatement certification or documentation required.
