29 CFR 1910.119(f)(1)(iii)(B): The employer did not develop and implement written operating procedures that included safety and health precautions necessary to prevent exposure, including engineering controls, administrative controls, and personal protective equipment. a) In the Machinery Room, on or about December 22, 2021 - The employer did not develop and implement written operating procedures that included safety and health precautions necessary to prevent exposure during the task of a high liquid level event in Recirculator vessel, Recirculator Operations, including engineering controls, administrative controls, and personal protective equipment. Failure to include safety and health precautions in operating procedures can expose employees to fire and toxic hazards. b) In the Machinery Room, on or about December 22, 2021 - The employer did not develop and implement written operating procedures that included safety and health precautions necessary to prevent exposure during the tasks of Manual Start-Up Procedures for the compressor and Manual Shut down procedures for the compressor, Compressor Operating Procedures and Deviation Recovery, including engineering controls, administrative controls, and personal protective equipment. Failure to include safety and health precautions in operating procedures can expose employees to fire and toxic hazards. c) On the Machinery Room Roof, on or about December 22, 2021 - The employer did not develop and implement written operating procedures that included safety and health precautions necessary to prevent exposure during the task of Manual Start-Up Procedures for the evaporative condensers, Evaporative Condensers, including engineering controls, administrative controls, and personal protective equipment.
