Machine Room - On or about December 14, 2021, the employer failed to develop written emergency shutdown procedures for the high pressure receivers that are used in the process of handling anhydrous ammonia, failure to establish accurate safe operating procedures for the ammonia system can lead to process upsets, and expose employees to fire, explosion and toxic hazards. b.
