29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition: a) Warehouse Department (Dock Area): On or about October 27, 2021, and times prior, the employer failed to take out of service a Yale Lift Truck (Model No.: GLC120VXNTSE092) and required employees to perform forklift operations with damaged tires and oil leaks; exposing employees to struck-by and crushed-by hazards during forklift operations.
