29 CFR 1910.28(b)(3)(ii): The employer did not ensure that each employee was protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems: (a) Lexmark International Inc. at 6555 Monarch Road in Longmont, CO: On and before July 2, 2021 the employer did not protect workers from stepping into a pit in the Anodization room, where the corner of the pit was unguarded where employees must walk to conduct work operations. Employees were exposed to trip and fall hazards.
