29 CFR 1910.28(b)(13)(ii): The employer did not ensure that when work is performed at least 6 feet (1.6 m) but less than 15 feet (4.6 m) from the roof edge, each employee is protected from falling by using a guardrail system, safety net system, travel restraint system, or personal fall arrest system: (a) Roof Tops above the Mechanical Room and Freezers: On or about 7-8-21 the employer did not ensure that employees performing daily work of ammonia system checks and regular maintenance when checking piping and equipment throughout the day when greater than 6 feet but less than 15 feet from the roof edge were protected from falls of greater than 10 feet by a guardrail system, safety net system, travel restraint system, or personal fall arrest system.
