29 CFR 1910.219(e)(1)(i): Where both runs of horizontal belts are seven (7) feet or less from the floor level, the guard shall extend to at least fifteen (15) inches above the belt or to a standard height, except that where both runs of a horizontal belt are 42 inches or less from the floor, the belt shall be fully enclosed in accordance with paragraphs (m) and (o) of this section. Employer is failing to protect employees from amputation and laceration hazards. This was most recently documented on March 17, 2021 in the facility located at 2900 C Street, Omaha, Nebraska 68107 in the milling area of the facility. Where the employer has employees operating, maintaining and adjusting the milling machines while exposed to inadequately guarded belts and pulleys. Instances in the Grinder Room of the facility include but are not limited to: i. 3 BKC #3 16 GET inadequate guard over the belt and pulley assembly. ii. 4 BKC #3 20 AD inadequate guard over the belt and pulley assembly iii. 1 M #5 36 STV inadequate guard over the belt and pulley assembly iv. 2 M #4 36 STV inadequate guard over the belt and pulley assembly v. 2 LG 36 STV inadequate guard over the belt and pulley assembly Instances in the 3rd Floor Mill of the facility include but are not limited to: vi. 3 BKF #2 18 AD inadequate guard over the belt and pulley assembly vii. 4 BKF #1 24 AD inadequate guard over the belt and pulley assembly
