29 CFR 1910.134(i)(8):An asphyxiating substance was introduced into breathing air lines: On or about February 4, 2021, the employer did not ensure that nitrogen could not be introduced into breathing air lines in that employees were required to hook up both nitrogen and shop air lines onto equipment simultaneously when performing nitrogen pressure testing on vessels, allowing nitrogen to backfill through the shop air line.
