29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: A) Custom Bandag, Inc.-8181 Route 130 South, Pennsauken, NJ: The employer failed to ensure that each employee who's work may have required the use of a Powered Industrial Truck (PIT's) was evaluated at least every three years as per the requirements of this standard. Condition noted on or about 01/20/2021. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
