29 CFR 1910.29(b)(2): The employer did not ensure that guardrail systems met the requirement that mid-rails, screens, mesh, intermediate vertical members, solid panels, or equivalent intermediate members that are installed between the walking-working surface and the top edge of the guardrail system as follows when there is not a wall or parapet that is at least 21 inches (53 cm) high: (a) Mount Saint Charles Academy - stairwell adjacent to the service freight elevator serving the school kitchen: On or about 1/11/21, the employer did not ensure that guardrail systems were provided with an adequate mid-rail exposing employees to fall hazards of up to 14 feet while using the stairs to access the freight elevator and basement kitchen: 1) The upper stairwell was missing a mid rail for half of its length. 2) The lower stairwell had no mid-rail.
