29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage and other deterioration, and defective components were not removed from service: (a)Brothers Construction, LLC @ 14648 423rd Avenue, Bristol, SD 57219: On or about and at times prior to September 1, 2020, the exposing employer did not ensure that each personal fall arrest system was inspected prior to use, and that defective components were removed from service. Employees were using a Guardian 50-foot Poly Steel Rope Vertical Lifeline assembly part #01320, as a part of the personal fall arrest systems used during the installation of steel corrugated roofing panels on a post-and-truss cattle barn. This condition exposed employees to a fall hazard of approximately 16 feet.
