Tri-State Garden Supply, Inc. dba Gardenscape, Inc., On or about September 2, 2020, at the address of U856 State Route 66, Archbold, Ohio 43502: In the Production Area, the employer did not provide guarding around a horizontal chain and sprocket on Hamer powered conveyor located on the Bagger # 1 Machine. Employee(s) operating on or near the unguarded chain and sprocket of the powered conveyor were exposed to caught in hazards.
