29 CFR 1910.119(j)(4)(iii): The frequency of inspections and tests of process equipment shall be consistent with applicable manufacturers' recommendations and good engineering practices, and more frequently if determined to be necessary by prior operating experience: At a facility located at 802 South Street, Plymouth, WI 53073; the employer did not ensure the frequency of independent inspections and/or tests of the ammonia refrigeration systems' piping and vessels. The employer's Mechanical Integrity policy referenced IIAR Bulletin No. 110 (1993) - "Start-up, Inspection and Maintenance of Ammonia Mechanical Refrigerating Systems", which required a competent person conduct examination and testing, as considered necessary by the competent person, at least every five (5) years.
