29 CFR 1926.652(c): Design of support systems, shield systems, and other protective systems. Designs of support systems shield systems, and other protective systems selected and constructed by the employer or his designee were not in accordance with the requirements of paragraph (c)(1); or in the alternative, paragraph (c)(2); or in the alternative, paragraph (c)(3); or in the alternative, paragraph (c)(4) as follows: At a trench located at 1501-1505 Gloryview Rd., Frisco, Texas; the employer did not design or utilize an adequate support system or a protective system for employees working inside an approximately 3 feet wide x 7 feet deep x 25 feet long trench. The employees were exposed to cave-in and crushed-by hazards.
