29 CFR 1910.28(b)(9)(iii): The employer must ensure ladder sections having a cage or well: Are offset from adjacent sections per 29 CFR 1910.28(b)(9)(iii)(A); and Have landing platforms provided at maximum intervals of 50 feet (15.2 m) per 29 CFR 1910.28(b)(9)(iii)(B). Lindenwood, East grain bucket elevator's vertical fixed ladder cage - on or about April 7, 2020, the employer failed provide at least one offset landing platform at the first 50 feet interval of a fixed ladder's run. Employees ascending/descending on a fixed ladder were exposed to fall hazards up to 100 feet likely to result in death or serious injury. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
